Registration Instructions

Welcome to Beloit Memorial High School Athletic Registration

Required information to successfully complete athletic registration are:

(Please have all information ready when registering, you will not be able to save and return)

  1. All students who plan to participate in athletics must watch the Athletic Code Meeting with a parent and complete the Athletic Code Online Meeting Survey prior to participating in a sport. Code Meeting will be available online starting August 14th. There will not be an in-person Code Meeting this year.
  2. Complete Athletic/Activities Registration Form
  3. REVIEW Student (Spanish) and Parent (Spanish) Concussion Information and COMPLETE Parents & Student Concussion Agreement
  4. Turn in completed WIAA Physical Form (Parent & Doctor’s signature required) into the BMHS Athletic Office or the BMHS Main Office. The WIAA has provided a Physical Extension Form that allow you to extend the your previous physical date, if you meet the criteria.

All fees need to be paid to the Beloit Memorial High School Main Office 

ALL FORMS MUST BE COMPLETED BY 1 WEEK PRIOR TO THE START DATE OF THE SELECTED SPORT TO ASSURE ELIGIBILITY TO TRYOUT OF PRACTICE ON THE FIRST DAY.

Students registering for Athletics must have a current physical examination on file in the Athletic Office before the student is authorized to participate in practice/tryouts.   To ensure clearance for participation, physicals should be completed and turned in to the Athletic Office no later than 3 school days prior to the first day of practice/tryouts.


2020-2021 WIAA PHYSICAL CARDS 

Physicals on file dated on or after April 1, 2019 have what is considered to be a current physical.  The appropriate WIAA physical card for a student with a current physical is the “Alternate Year” phycial card.  Students who have not had a physical since April 1, 2019 will need to schedule a physical prior to registering for their sport.  The correct WIAA physical card for a student who needs a physical is the green “Physical” card.  When a student registers for their first sport of the school year, the appropriate WIAA physical card must be submitted to the Athletics Office, along with the Athletics Registration.  If the correct form is not submitted with the Athletic Registration form, the student will not be allowed to practice.
